Out of Zone PTS Form

This form is used by players who want to participate in the Lakers try-outs but are from a Zone or Centre that has Fall Try-outs. However, it can only be used by an ALLIANCE Hockey player who did not participate or play in a AAA center or zone the previous season. This form is not a release. The participant must attend the try-outs in their home centre or zone in the fall and must obtain an OHF AAA Waiver to be eligible to sign with the Lakers. Please speak to your Lakers coach ASAP if you believe this situation applies to you.
