All Teams applying for this tournament MUST complete this form and then send a registration fee cheque in the amount of $1250.00 USD/CAD payable to Huron Perth Lakers Tournament to the following address in order to ensure consideration: Attn: Great Lakes Cup, Huron Perth Lakers Hockey Association 75 Waterloo Street, Stratford, Ontario, Canada P.O. Box 1052, N5A 4A0. Deadline is July 31, 2014. All teams must be a member of Hockey Canada or USA Hockey. 3. Accepted teams when contacted will be required to submit roster and travel permit (permission from governing body)
